Abstract: Introduction: To address the public health challenges related to the preservation of expelled immature permanent teeth, we have pushed our research towards alternative plant-based media. Plants have always been vital to humanity. This is the case of Aloe which is known for its great diversity with different biological activities. The species chosen for our study is Aloe schweinfurthii. The purpose of the survey was to conduct research on cost-to-dose, accessibility, efficacy and safety. Our overall goal was to evaluate the toxicity of Aloe schweinfurthii gel in order to safely ensure the survival of periodontal ligament cells from an expelled immature permanent tooth. Methods: This experimental study involved acute and subacute toxicity. We evaluated its effects on the weight parameters of the animals, liver, kidney and hematological as well as the histopathology of the organs. Statistical analyses were done by SPSS version 23.0. The tests performed were ANOVA, Student and Newman-Keuls. The values of p <0.05 were considered statistically significant in all tests. The figures were made by Graph pad prism version 8.0.1. Results: The administered A. schweinfurthii did not cause any animal death, nor did they show any obvious sign of toxicity. Weight gain in rats in acute and subacute toxicity was observed. A decrease in liver, lung and brain weight was noted. We noted also an increase in the rate of ALAT, ASAT, serum creatinine, serum uric acid, white blood cells at satellite doses. Histopathology of the organs showed no cellular disorganization, nor damage to the hepatic and renal tissues. Preservation of cellular vitality was observed at all minimum concentrations from 24 to 72 hours in complete safety. Conclusion: Acute and subacute toxicity tests were unremarkable. A. schweinfurthii gel appears to be a good medium for the safe preservation of periodontal ligament cells.

A Prospective Clinical Study on Transgingival Lag Screws for Management of Dentoalveolar Fractures

Abstract: Dentoalveolar fracture (DAF) is a common injury which comprises of alveolar fractures which include 2-8% of facial fractures. Traditional methods of management of these fractures may not be feasible all the time. The aim of this study is to evaluate the efficiency of transgingival lag screws (TGLS) in management of dentoalveolar fractures. 20 patients with dentoalveolar fractures, either isolated or concomitant with other facial fractures were included in the study. All patients were treated using transgingival lagscrews under local or general anaesthesia and were evaluated for stability, reduction, bone loss, periodontal health and functional rehabilitation with a follow up period of 3 months. All the subjects healed without any complications. The screws were retrieved by the end of 3rd month. A descriptive statistical analysis was done for the assessed parameters. Good fracture stability was noted in 90% of the cases in immediate post operative period. Anatomical reduction assessed radiographically was ideal. Sound periodontal health was maintained effortlessly, inversely reflecting on minimal bone loss. Rehabilitative period was minimal with preinjury average bite force establishment by the end of 3 months. The study concluded that TGLS is an effective alternative to achieve reduction and stabilization in management of dentoalveolar fractures without jeopardizing the blood supply.

Abstract: Purpose: This review aimed to synthesize knowledge from the dental literature regarding the effects of the shortened dental arch (SDA) concept on patients' oral health-related quality of life (OHRQoL). Materials and Methods: A comprehensive search of English-language, peer-reviewed literature on the SDA was conducted using the Medline (PubMed), Embase (OVID), Scopus, and Google Scholar databases. Studies that evaluated oral health-related quality of life as an outcome were critically appraised.Results: The literature reviewed indicates that patients with SDAs do not exhibit a statistically significant difference in overall OHRQoL scores compared to other study groups. Conclusion: When determining the most appropriate prosthodontic treatment for partially edentulous patients, it is essential to consider patient expectations and preferences. A fully rehabilitated dental arch does not always guarantee a successful outcome. Modern treatment approaches should prioritize function, patient satisfaction, and overall well-being rather than solely focusing on complete dentition restoration. The SDA concept offers significant advantages and may serve as a viable alternative to reduce the need for extensive restorative treatment in the posterior regions of the mouth.

Outcome Analysis of Different Plating Positions in Mandibular Angle Fractures: A Prospective Randomized Study

Abstract: Background/Aim: This is a prospective Randomized study which aims to evaluate the rehabilitation period and functional outcomes associated with three distinct plating positions superior border lateral border and inferior border in cases of favorable mandibular angle fractures. Materials and Methods: Clinical and radiological evaluations were conducted on a sample size of 30 patients with favorable mandibular angle fractures before surgery during the first week at first month and three months post operation. Clinical outcomes, specifically the bite force for the three different plating positions were measured using a bite force device. Functional outcomes were assessed through Maximal Interincisal Opening (MIO), occlusion, neurosensory deficits and radiographic assessment of fracture reduction using pre-operative and post operative radio graphs i.e. OPG. Results: The superior border plating group exhibited a notable enhancement in bite force after surgery when compared to the other two groups with p-value being significant (p<0.001). Post operatively the inter-group comparison revealed no significant difference pre-operatively (p=0.039) but post-operatively (p<0.001) was significant. The inferior border plating group experienced a 30% rate of neurosensory deficits after the operation unlike the other two groups. Occlusal discrepancies were observed in 10% of the inferior border plating group while there was no significant difference in radio graphic reduction among the three plating groups. Conclusion: The study concludes that superior border plating is the most effective plate position for facilitating prompt recovery and minimal complications providing sufficient stability for favorable angle fractures.

Abstract: Background: Impaired localized epithelial immune system leads to oral cancer. Langerhans Cells (LCs) act as regulators of immune response which are regulated by Transforming Growth Factor β. But their association in a potentially malignant disorder like Oral Submucous Fibrosis (OSF), has not been researched enough. The aim of this paper was to assess the relationship between TGF β and LCs in OSF and whether TGF β can be successfully used as a therapeutic target. Context: TGF β has been a known regulator of immune response through its action on LCs which is proven in some in –vitro studies and also some of the clinical trials targeting TGF β. Aims: The aim of this paper was to assess the relationship between TGF β and LCs in OSF and whether TGF β can be successfully used as a therapeutic target. Methods & Material: Forty OSF and nine normal buccal mucosa were sectioned and subsequently stained immunohistochemically stained anti CD1a and anti (CD105) antibody respectively. Statistical Analysis Used: Student’s-t-test and One way ANOVA was applied. Results: Overall reduction of LCs in the epithelium of OSF than normal mucosa. There was no association seen between the expression of TGFβ and LCs in different stages or grades of OSF or in normal mucosa. Conclusion: Reduced number of LCs in OSF indicates that it does not correlate with the expression of TGF β. Areca itself may have cytotoxic/genotoxic effects on LCs.

Abstract: Background Concurrent nutcracker syndrome, superior mesenteric artery syndrome, and median arcuate ligament syndrome (the clinical expression of which is referred to celiac artery compression syndrome), is an extremely rare phenomenon. The purpose of this case report is to describe the anesthetic management of a patient who presented with this rare concurrence, for removal of third molar teeth under general anesthesia. The patient additionally had a history of problematic reactions to benzodiazepines, which further complicated his anesthetic management. Case Presentation A 16-year-old male presented for removal of his symptomatic third molar teeth. Comprehensive history and examination was performed, and medical clarification from his physician was obtained. The patient was treated in an outpatient setting with an anesthetic technique that avoided the use of nitrous oxide and any benzodiazepine, and which included oxygen, fentanyl, decadron, propofol, and local anesthesia. Postoperative antibiotics and pain medications were prescribed. Comprehensive post-operative instructions stressed the importance of postural requirements and adequate nutrition. The patient tolerated the procedure well, and experienced an uneventful recovery. Conclusion This case report illustrates the importance of comprehensive preoperative evaluation, including consultation with any treating physicians of record. It also serves to highlight the efficacy of total intravenous anesthesia, without the use of benzodiazepines or nitrous oxide. The importance of postoperative postural and nutritional requirements are critical with respect to the treatment of patients with any concurrence of the syndromes described in this report.
