Instrumental Extraction by Obstetric Suction Cup at Nabil Choucair Health Center (Dakar, Senegal) from 2005 to 2016: Indications and Prognosis

Abstract: The objective of this study was to take stock of instrumental extractions using an obstetric suction cup at the Nabil Choucair Health Center in Dakar. It was a retrospective, descriptive study on instrumental extractions by vacuum cup from January 1, 2010 to December 31, 2017 (144 months) at the maternity unit of the Nabil Choucair Health Center. This study included all patients who had experienced vacuum assisted delivery. The parameters studied concerned socio-demographic, clinical characteristics, indications and maternal-foetal complications. The collection was carried out using the fact sheet completed on the basis of the analysis of the files, the delivery register, and the anaesthesia register of the operating room. Data entry was carried out using Sphinx version 5 software and data analysis by Epi info version 3.5 software and descriptive analysis was carried out by studying the frequency, the mean and the standard deviation. During the study period 66,000 women gave birth and we collected 360 instrumental extractions. The frequency of the vacuum cup was 0.5% for instrumental extractions. The average age of the patients was 24.5 years, parity was 1.6. The average gestational age was 39.4 Weeks of Amenorrhea (SA), the average uterine height was 33 cm, the noises of the foetal heart were normal in 98% of parturients. The indications were dominated by a stop in the progression of foetal mobile (39%). The mean average birth weight was 2994 g and 97% of new-borns had an Apgar score of 9. Maternal complications were dominated by simple perineal tears (7%) and 3 cervical tears (1.2%). The neonatal prognosis was dominated by 13 caput succedaneum (chignon) (7%). The obstetrics suction cup is still decisive for the foetal outcome in our maternities. The mastery of the technique and the right indications help to avoid complications.

Abstract: The physical and psychosocial problems associated with obstetric fistula affect the patients’ efficacy to function within specific realms of life and affect their quality of life. This study documented the obstetric fistula patients’ self-efficacy for reacceptance in the communities in Africa. The study used keywords and the year of publication (2000 to 2019) as a search strategy to obtain data for review. A Critical Appraisal Skills Programme checklist specifically for qualitative studies was used. The search resulted in 284 studies: Google Scholar; 37, PubMed; 12, Hinari; 3, African Journals Online; 36, Psych-INFO; 1 and other websites; 195. Subsequently, 258 studies were excluded due to duplication and failure to meet the study objective leaving 26 studies, which were thematically analyzed. Studies report loss of hope, dignity, confidence, and self-esteem among obstetric fistula patients. They feel unfit, and are often anxious, attributes that impede their employability, mobility, connections, conjugal affairs, childbearing, and enthusiasm to accomplish various tasks; an index of low self-efficacy. However, when repaired and empowered through skills training and counseling, their self-efficacy improves. Self-efficacy is generally low among obstetric fistula patients especially those that have not yet had fistula repair. Alongside repair of the fistula, activities intended for societal reintegration reduce levels of anxiety, increase confidence and overall self-efficacy, which enables reacceptance.

Abstract: Objective: To evaluate the influence of individualized nursing care on Chinese postpartum women. Methods: We invited 182 participants to join our study, who are childbirth in the First Affiliated Hospital of Jinan University, their childbirth time is from April 2016 to October 2019. They were randomly assigned to control group (n=91) and intervention group (n=91), the two group receive different nursing services, such as individualized nursing care and traditional nursing services. We use Self-Rating Anxiety Scale (SAS), Self-rating depression scale (SDS) and other questionnaires to collect the data from participants, the data include patient comfort status, patient satisfaction status, anxiety status and depression status. Result: In comfort research, the comfort status of most patient is 0 level in control group and intervention group (n=44 & n=56). On the other hand, the control group had more 1 level assessment in this result (n=22). In satisfaction report, the performance of individualized nursing care is better than traditional nursing services in satisfaction [86 (94.5%) vs 77 (84.6%)]. In SDS and SAS research, The intervention group had greater improvement than that of control group in the SAS as the result of intervention group lowered scores even more. Conclusion: the individualized nursing care have great improvement to improve hospital satisfaction and mental health of patients. But the individualized nursing care has little effect on improving patient comfort.

Abstract: Objective: To assess the influence of the new advanced nursing role services on postpartum women. Methods: 162 patients to join our study, who undergoing spontaneous labor or cesarean in the First Affiliated Hospital of Jinan University. Their time of spontaneous labor or cesarean is from February 2017 to January 2020. They were randomly assigned to the control group (n=81) and an intervention group (n=81), that control group receive traditional nursing services and intervention group receive the new advanced nursing role services. Our researchers collect the data from participants by Self-Rating Anxiety Scale (SAS), Self-rating depression scale (SDS) and the questionnaire which patients reported access to supportive care resources, the data include supportive care resources status, satisfaction status, anxiety status and depression status. Result: In supportive care resources report, more patients of intervention group think their receive nursing service from the health care provider outside office time [44 (54.3%) vs 56 (69.1%)]. In satisfaction report, intervention group had more assessing of very well than that of control group (n=67 vs n=44). In Self-Rating Anxiety Scale, Self-rating depression scale, the intervention group had greater improvement in anxiety status than that of control group (from 62.4±7.7 to 51.8±6.1 vs from 63.4±7.2 to 57.4±6.9). Conclusion: The new advanced nursing role services provide great improvement of mental health in postpartum. However, the new advanced nursing role services need more expensive cost, but most Chinese patients are not able to afford this new nursing services.
