Abstract: Maternal mortality is the highest in sub- Saharan Africa, where the life time risk of death from pregnancy related conditions is 1 in 16, compared with 1 in 2800 in developed countries, In Ethiopia, like other sub Saharan Africa countries, maternal and infant mortality remains to be a problem of public health importance. There are 350 maternal deaths for every 100000 live births. Antenatal care, the care that the women receive during pregnancy, helps to ensure healthy outcome for women and new born. A community based cross sectional study design was conducted. Systematic random sampling technique was used. A total of 345 women who delivered in the 5 years before the date of survey were interviewed. Two hundred twenty three (64.6%) women were reported to have received antenatal care at least once during their last pregnancy. Of these, 89 (39.9%) had four or more antenatal care follow ups during their last pregnancy. This is low coverage compared to other developing country. The government sector and health worker need to give great attention for antenatal care.

Abstract: Insecticide Treated bed Net (ITN) is considered to be the most efficacious of all currently feasible interventions for malaria control in Africa. However, lack of data on its use in under five children has made it difficult to keep tract of its efficacy in this population. This study was aimed at pooling published data from parts of Africa and Asia with the hope of providing an overview of the use of ITNs and its efficacy in children under five years of age. Literature search was carried out Pubmed, Pubmedcentral, MEDLINE, and Cochrane library for all published articles between 1991 and 2015 using search strings such as under five children, ITN, malaria parasitaemia, anaemia, protective efficacy, ownership and use, pattern of deployment and types of ITN. Based on specific criteria, 30 studies were included. Few countries have met the ITN coverage target of 80% including Nigeria, with coverage rates ranging from about 69% to 75%. ITN use in under five children has been shown to reduce the prevalence of malaria and anaemia in areas unstable malaria transmission to a variable degree. ITN possession does not always translate to usage and consistent use is required for effective malaria control.

Abstract: Some mothers have negative ideas about human milk donation, especially in the Muslim population, because of the «milk brother-sister» concept, whereas infants are often in need of human milk. A mother delivered a very low birth-weight infant of 500 g after 26 weeks of amenorrhea. She was able to collect 200 mL of milk per day by four days after birth, 500ml/day after seven, and then up to 1.5 L/day. At the end of the hospitalization, the milk bank asked her if she was willing to donate her milk. She first refused to offer her milk for donation because of the «brother milk» concept. After discussion, the milk-bank team managed to convince the mother to donate the 179 Liters of milk not used by her baby. The two issues that may arise for a Muslim are whether they allowed to donate their own milk and whether their infant can receive donated milk. These issues were addressed by: i) performing a literature review covering all points of view of the religion concerning human milk donation, ii) seeking the expertise of religious figures, and iii) examining biological and genetic issues. Thus, religious, cultural, biological, and epigenetic aspects all support milk donation by Muslim mothers to milk banks and allow children to receive donated milk. Milk banks should be created in Muslim countries to promote the health of pre-term infants.

Abstract: Pneumonia is a major cause of morbidity and mortality in children especially in developing countaries. Lung ultrasonography (LUS) is an accepted safe diagnostic tool with many advantages over chest x-ray (CXR). The objective of this prospective observational study was to evaluate the usefulness of LUS in comparison with CXR for diagnosing pneumonia in 60 children presented with fever and respiratory distress. For each patient complete diagnostic workup was performed including history, physical examination, routine laboratory investigations, CXR and LUS. LUS was performed by a sonographer blinded to patient’s clinical and CXR findings. The patients were classified into; Pneumonia group: included 45 patients diagnosed by two clinicians as having community acquired pneumonia (CAP) based on the recommendations of British Thoracic Society and Non pneumonia group: included 15 patients received other diagnoses. The clinicians were blinded to CXR and LUS findings. In pneumonia group, sonographic and radiographic findings compatible with pneumonia were found in 95.6% and 88.9% of patients respectively. In non pneumonia group, LUS can exclude pneumonia in 93.3% of patients while CXR excluded pneumonia in 86.7% of them. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of LUS to demonstrate lung involvement were higher than those of CXR (95.6%, 93.3%, 97.7%, 85.7% and 94.5% versus 88.9%, 86.7%, 95.2%, 72.2% and 86.7% respectively). LUS was superior to CXR in identifying pleural effusion and multiple consolidation in more than one lobe. Being safer, cheaper and relatively more time saving, lung ultrasonography offers an important contribution to the diagnosis of CAP in febrile children with respiratory distress compared to chest X-ray.
